Abstract

Chronic diseases like hypertension, heart failure or diabetes are common among the elderly. Patients suffering from those and other diseases face a reduced quality of life and a high risk for severe events like myocardial infarction or stroke. Such events often lead to severe disabilities and dramatically change the lives of the affected persons.
